Collaborative family practice teams have doctors, nurse practitioners, family practice nurses and other health providers working together to provide primary care in communities.
We know that accessing a family doctor or nurse practitioner can be a challenge for some people. Nova Scotia Health Authority is creating more and strengthening existing collaborative family practice teams across the province to improve access to the primary care you need.
